Who is responsible

What we collect

Only the professional contact details you enter in the application form: name, professional email, country, city, institution or practice, specialty, website, approximate surgical volume, your answers about structured risk assessment and demonstrations, your comments, your area of interest and your consent. To prevent abuse we also keep a one-way hash of your IP address and your browser's user-agent string.

The form does not ask for patient data or any health information. Please do not include patient information in the comments.

How we use it

Only to review your application and to contact you about it (clinical implementation or multicenter research collaboration). You will not be added to newsletters or marketing campaigns, and your data is not sold or shared with third parties for their own purposes.

Where it is processed

• Supabase (server function and database, United States), where applications are stored with no public access.
• Resend (email delivery, United States), which delivers the application to Dr. Bukret by email.
• GitHub Pages (hosting of this website).

Retention and your rights

Applications are kept while the application or the resulting collaboration is being evaluated. You can ask to access, correct or delete your data at any time by writing to drbukret@drbukret.com from the email address you used in the form.

This notice covers only the application form. The use of the AIRA Adaptive software itself is governed by the AIRA Adaptive Privacy Policy.
